Home Address Ltd is a small, family-run care service based in Fishguard, Pembrokeshire. We believe that everyone deserves to live in a safe, comfortable and supportive environment where individuality is celebrated and independence is encouraged.

Gofal Preswyl - Tŷ Vergam
Mae Tŷ Vergam yn gartref preswyl â chwech ystafell wely yng nghanol tref Abergwaun. Rydym yn cynnig lleoliadau tymor hir yn ogystal â gofal seibiant byr. Mae ein staff profiadol yn darparu gofal personol a chefnogaeth i bobl ag anableddau dysgu, epilepsi ac anghenion cymhleth. Rydym yn gweithio’n agos gyda gweithwyr iechyd proffesiynol i sicrhau bod pob preswylydd yn derbyn gofal personol cyson mewn amgylchedd cartrefol a diogel.

Byw â Chymorth
Mae ein tîm byw â chymorth yn cefnogi oedolion sy’n byw yn eu cartrefi eu hunain ledled Sir Benfro. Rydym yn cynnig pecynnau gofal hyblyg o ansawdd uchel sydd wedi’u cynllunio i hyrwyddo annibyniaeth, urddas a dewis. Gall ein tîm ddarparu rhwng 50 a 250 awr o ofal yr wythnos, gan gynnig cymorth gydag anghenion bob dydd, mynediad i’r gymuned, gofal personol a chymheiriaid caredig.

We provide a range of care and support options designed to meet the individual needs of adults with learning disabilities.

Residential Care - Vergam House
Vergam House is a six-bedroom residential home in the heart of Fishguard. We offer long-term placements as well as short-term respite care. Our experienced staff provide personal care and support for people with learning disabilities, epilepsy and complex needs. We work closely with health professionals to ensure each resident receives consistent, person-centred care in a safe and homely environment.

Supported Living
Our supported living team supports adults living in their own homes across Pembrokeshire. We offer flexible, high-quality care packages designed to promote independence, dignity and choice. Our team can provide between 50 and 250 hours of care per week, supporting daily living, community access, personal care and companionship.
