The following need to completed prior to discharge of patients in ICN-3:

1. Newborn Metabolic Screens (#1, #2, and #3) according to the Newborn Screening Guidelines: _

2. Hearing Screen ordered as Newborn Hearing Screen a couple of days prior to discharge

3. Critical Congenital Heart Disease Screen according to Critical Congenital Heart Disease Guideline. If infant had an Echo this does not need to be done.

4. Car Seat Trial and passed at least the day before discharge

5. Parents complete discharge education

6. Home Visitation Referral should be offered to all families that are not established with HATCH or FOCUS. Once filled out with family please give to our Patient Care Tech to fax the form

7. All breastfeeding Moms should be offered follow up with lactation. Our lactation specialists can assist to find appropriate follow up for patients that reside outside of Albuquerque

8. PCP appointment needs to be scheduled, PCP updated, and fax number obtained to fax finalized discharge summary

9. Discharge Summary completed and provided to parents prior to discharge (2 copies) ***Discharge summaries should not be distributed to anyone other than biological parents***

10. Depart Process completed (NICU discharge form) updated with discharge diagnosis and follow up appointments
