No more blue sheets
Every provider has an assigned MSA. This MSA will go through each clinic list the following week, look at the follow-up note and schedule f/u appointments accordingly. They will notify the care manager RN if there is anything outside of scheduling a f/u visit that is needed (such as interim labs)
To make it clear to the MSA what the f/u is, put a “Follow Up:____” paragraph at the end of the note with clear instructions.
If there is anything needed in 4 weeks or less (RV, interim labs, imaging, etc…), email the care manager. These would be:
Oncology: Nickey Hosler at Nicole.Hosler@va.gov
Hematology: Currently Elaine Romero at Eunice.Romero@va.gov. Will soon be Sarah Bess at Sarah.Bess@va.gov
Emails don’t have to be detailed, if anything you can copy and paste what’s in your note or even something as simple as “please see my note for Mr.___ (last 4) for time sensitive f/u”
Please refrain from cosigning if at all possible, especially the MSAs. The MSAs are cosigned to dozens of notes right now (due to former process for f/u) and I guarantee they will not see it in a timely fashion.

DIRECT ADMITS FOR CHEMO ONLY (ONCOLOGY) (updated 7/2020)
Direct admits for chemo only do not need to be seen, but to help out the primary team (who often think onc will automatically see them or may have questions), the fellow should put a brief note in the chart as outlined below. You should identify the hematology or oncology care manager as an additional co-signer on this note to help her keep track of these patients.
Mr(s). X will be receiving ____chemo regimen today. He/She does not require an oncology consult . Chemo/Immunotherapy orders have been signed and he/she was cleared for chemotherapy at the visit on ____(date). He/she can be discharged after chemotherapy is complete. He/she has a follow up on ___(date)in our clinic.Please discharge the patient with ____ (anti-emetics, steroids, etc).
