Funded by MassHealth (Medicaid), Adult Foster Care allows a family member or friend to provide 24-hour care for qualified members in their own homes. Caregivers receive a tax-free payment for their services, to help with the burden of care. Adult Foster Care services through MassHealth could be the solution for adults unable to live on their own who would like to retain the dignity and freedom of living at home.

Who is eligible for AFC ?
Recipients that are:
16 years of age and older.
Have MassHealth Standard, CommonHealth, Senior Care Options (SCO), OneCare, or PACE.
Need help with Activities of Daily Living (ADLs) such as bathing, transfering, dressing, eating, toileting, mobility, and other elements of personal care.
A registered nurse and care manager will conduct an assessment to determine eligibility. The Ubuntu Care AFC Program provides financial reimbursement to family members/caregivers. Services are provided 24 hours a day, 7 days a week by the family member/caregiver to the AFC recipient living in their home.
