Please fill out the Health History form in the Day Camp Registration packet with the medications that will be brought to camp.

Before coming to camp, place the medication in its original packaging in a large gallon ziplock bag with the camper's name on it. Print, fill out, and enclose the "Request for Medication" Form in the bag as well.
If medication must be dropped off, deliver it to the camp nurse during drop off in the morning.
