ThriveWell Community Health Foundation was created with a simple belief at its core: every person deserves access to compassionate, quality healthcare, no matter their income, background, or circumstances. Too many families in Immokalee and surrounding communities struggle to get even basic medical or mental health support. Our foundation exists to bring hope, dignity, and care to those who need it most.

Our Mission

Our mission is to expand access to essential healthcare for medically underserved individuals and families across Florida. We work to remove financial barriers to care by partnering with trusted local clinics and providers who share our commitment to serving the community. Through these partnerships, we help fund medical visits, mental health services, labs, screenings, and other vital resources that promote health and healing.

Our Purpose

Our purpose is to stand in the gap for people who are too often overlooked. We aim to ensure that no one has to choose between their health and their ability to pay. By supporting individuals in need, we strengthen families, uplift communities, and promote long-term well-being. Programs and Future Goals

ThriveWell Community Health Foundation is committed to creating meaningful, long term change for medically underserved individuals and families in Immokalee and surrounding Florida communities. Our programs are designed to remove barriers to care and support the whole person, not just the illness. With community support and future funding, we aim to grow in several key areas.

 Our planned programs include:

• Expanded access to medical care, ensuring that individuals can receive primary care visits, chronic disease management, labs, and screenings regardless of their ability to pay
• Mental health support services, helping community members of all ages access counseling, evaluations, and emotional support during times of need
• Preventive and wellness programs, such as nutrition education, fitness and activity initiatives, early detection screenings, and chronic disease prevention
• Community education and outreach, including workshops on health literacy, self care, mental wellness, disease prevention, and navigating available local resources
• Partnership-based care, collaborating with local clinics, outreach organizations, and community groups to ensure that underserved families receive timely and coordinated support
• Support for rural and vulnerable populations, addressing unique barriers such as transportation, language, and access to consistent primary care
