Get With the Guidelines Stroke Gold Award:

Hospitals receiving Get With The Guidelines® Gold Achievement Award have reached an aggressive goal of treating patients with 85% or higher compliance to core standard levels of care as outlined by the American Heart Association® for two consecutive calendar years.

Get With the Guidelines Stroke Gold Plus Award:

Hospitals receiving Get With The Guidelines® Gold Plus Achievement Award have reached an aggressive goal of treating patients with 85% or higher compliance to core standard levels

of care as outlined by the American Heart Association® for two consecutive calendar years. In addition, those hospitals have demonstrated 75% compliance to (seven out of 10 stroke quality measures) during the 12-month period.

Target: Stroke HONOR ROLLS

There are 4 levels of Target Stroke awards.


Target: Stroke Honor Roll:

Time to thrombolytic therapy within 60 minutes in 75% or more of acute ischemic stroke patients treated with IV-tPA.

Award Years: 2011, 2012, 2013, 2014


Target: Stroke Honor Roll-Elite:

Time to thrombolytic therapy within 60 minutes in 85% or more of acute ischemic stroke patients treated with IV-tPA.

Award Years: 2015, 2022


Target: Stroke Honor Roll-Elite Plus:

Time to thrombolytic therapy within 45 minutes in 75% or more of acute ischemic stroke patients treated with IV-tPA AND door-to-needle time within 30 minutes in 50% of acute ischemic stroke patients treated with IV-tPA.

Award Years: 2016, 2017, 2018, 2019, 2020, 2021, 2022


Target: Stroke Honor Roll Advanced Therapy:

Door-to-device times in at least 50% of applicable patients within 90 minutes for direct arriving and within 60 minutes for transfers.

Award Years: 2020, 2021, 2022

Target: Type 2 Diabetes HONOR ROLL

Type 2 Diabetes Honor Roll:

This award is a distinguished honor for hospitals engaging in advanced care for patients with heart failure and/or stroke and type 2 diabetes. This award allows Get with the Guidelines® participating hospitals to be acknowledged for their compliance with Get with the Guidelines measures tailored to patients with type 2 diabetes. The hospital must demonstrate 90% compliance for 12 consecutive months (no matter if award is Silver or Gold) for the “Overall Diabetes Cardiovascular Initiative Composite Score” measure in the selected module.

Award Years: 2019, 2020, 2021, 2022


Measures:

  • Percent of acute ischemic stroke patients with diabetes who arrive at the hospital within 210 minutes (3.5 hours) of time last known well and for whom IV t-PA was initiated at this hospital within 270 minutes (4.5 hours) of time last known well.

  • Percent of patients with ischemic stroke or TIA and diabetes who receive antithrombotic therapy by the end of hospital day two.

  • Percent of patients with diabetes and an ischemic stroke, or a hemorrhagic stroke, or stroke not otherwise specified who receive VTE prophylaxis the day of or the day after hospital admission.

  • Percent of patients with an ischemic stroke or TIA and diabetes prescribed antithrombotic therapy at discharge.

  • Percent of patients with an ischemic stroke or TIA with atrial fibrillation/flutter and diabetes discharged on anticoagulation therapy.

  • Percent of patients with ischemic or hemorrhagic stroke or TIA and diabetes and a history of smoking cigarettes, who are, or whose caregivers are, given smoking cessation advice or counseling during hospital stay.

  • Percent of ischemic stroke or TIA patients with diabetes who are prescribed high-intensity statin therapy at discharge OR, if >75 years of age, are prescribed at least moderate-intensity statin therapy at discharge.

  • Percent of diabetic patients or newly-diagnosed diabetics receiving diabetes treatment in the form of glycemic control (diet or medication) or follow-up appointment for diabetes management scheduled at discharge.

  • Percent of ischemic stroke or TIA patients with diabetes who receive therapeutic lifestyle recommendations at discharge.

Achievement Measures AWARDS

All Achievement measures must be met at least 85% of the time in order to qualify for an award (Gold, Silver, Bronze).

Bronze: A period of 1 quarter (90 days) and can only be earned once.

Silver: Recognizes performance on 12 consecutive months (calendar year).

Gold: Recognizes performance on 24 consecutive months (calendar year).


Measures:

  1. Percent of acute ischemic stroke patients who arrive at the hospital within 210 minutes (3.5 hours) of time last known well and for whom IV t-PA was initiated at this hospital within 270 minutes (4.5 hours) of time last known well.

Quality Measures AWARDS

Quality measures (at least 4 of the 6) must be met at least 75% of the time in order to qualify for the plus portion of the award (note that a hospital does not have to earn a plus portion of the award to earn a Gold, Silver, or Bronze award).

Bronze: A period of 1 quarter (90 days) and can only be earned once.

Silver: Recognizes performance on 12 consecutive months (calendar year).

Gold: Recognizes performance on 24 consecutive months (calendar year).


Measures:

  1. Percent of stroke patients who undergo screening for dysphagia with an evidence-based bedside testing protocol approved by the hospital before being given any food, fluids, or medication by mouth.

  2. Percent of patients with stroke or TIA or their caregivers who were given education and/or educational materials during the hospital stay address all of the following: personal risk factors for stroke, warning signs for stroke, activation of emergency medical system, need for follow-up after discharge, and medications prescribed.

  3. Percent of patients with stroke who were assessed for rehabilitation services.

  4. Percent of acute ischemic stroke patients receiving IV-tPA during the hospital stay who have a time from hospital arrival to initiation of IV-tPA (door-to-needle time) of 60 minutes or less.

  5. Percent of ischemic stroke or TIA patients with a documented lipid profile.

  6. Percent of ischemic stroke and stroke not otherwise specified patients with a score reported for NIH Stroke Scale (initial).
