The below links will forward you directly to the form which needs to be completed in order to have your information blocked, A majority of the links on this page are held via The Clerk of Court. Under Florida Statute 119.071 these agencies will comply with the removal of your information due to your employment status. They generally will cover your spouse and children in some circumstances but it is important to remember that not every situation is the same.

Hillsborough County has the same voter information removal (which gets sent to their Falkenberg location, listed on the form. Unfortunately the format we have it in does not comply with the google parameters so even after turning it into a PDF it is not able to be viewed. We do have a member that is a Notary Public and will notarize any forms you need done (including those for friends and family) free of charge.
