Appointment Policies and Procedures

1) To limit the amount of people coming into the office, we ask that all appointments be made over the phone or online at calendly.com/tmoptometry

2) If you can not fill out the patient form before coming in, PLEASE come in 10 minutes earlier to fill out paperwork.  If you are late to your visit, we may not be able to accomodate you in our schedule.

3) Contact Lens Patients: Please come in wearing your glasses to limit the amount of times you have to touch your eyes. 

4)New Patients: Please bring any information regarding your last exam or bring in your glasses (even if they're broken) / contact lens boxes (or foil wrapping) to help expedite your visit.

5) We must have your vision insurance information at least 24hrs prior to your visit. If we do not have your information, you will need to pay out of pocket and submit your receipt directly to your provider (We will provide an itemized receipt ). The main reason is that we may not be in your plan's network and we do not want to waste your time by coming in, then being told we are not in your network. We are also trying to keep our promise to keep all visits on time.
