This case study was written about a doctor who wrote out a prescribed medication incorrectly, and although double checked several times, the error still managed to occur. Thankfully, no one was injured in this case study. The patient just had side effects of oral steroids and the error was quickly caught.

This case study has a more upsetting ending. The little girl Josie whose parents trusted medical staff after suffering burns suffered a painful death from an overdose of opioids. Her mother advocated for her and was continuously ignored by her caregivers in the hospital. Because of this, these hospital workers took something away from that mother she can never get back.
