The game is a modified Russian roulette version played in an underground nightclub with a mysterious "Dealer" using a pump action shotgun. The game has three rounds, with the Dealer loading the shotgun with red live shells and blue blanks randomly. The player chooses to shoot the Dealer or themselves, and if the shell is live, either the player or Dealer loses a life. If the shell is blank, the player continues their turn or forfeits the shotgun to the Dealer.
