At risk of becoming homeless
Transitioning from a licensed setting
Eligible for MA waiver services
Needs assistance due to their mental health condition to:
Communicate
Manage challenging behaviors
Make decisions
Provider Referral Information
Referrals for Housing Stabilization Services can be made by a Targeted Case Manager, Waiver Case Manager or Housing Services Consultant. If you would like to refer an individual to our program, please complete the Referral Form.
