1) PHYSICAL: Make sure your child has a current physical on file.

NOTE: if they had their physical done at school previously, the school will not be doing physicals onsite this year. Don’t wait until last minute to get it done.

2) PREPARTICIPATION PHYSICAL FORM: click on the link above for the form you will need to take to the doctor for a signature. FYI, some doctors already have their own form (ie Kaiser).

3) ATHLETIC CLEARANCE: click on the link above, or copy and paste this link into your browser: https://athleticclearance.com/login.php

- Log in and Click “Start Clearance Here”

- Select “2020-21” for the year

- Select “Temple City (CIF-SS)” for the School

- Select “[your child's sport team]”, click submit

- Select your child’s name from the drop down menu. Prior information should auto populate.

- Select the grade of your child for the fall

- Scroll down to Insurance information

- Select “Yes” or “No”

- Confirm that your insurance info is listed correctly

- Scroll down to “Education History” and select the appropriate option, click “Save”

- Upload a current physical, NOTE: if your child's physical has not expired, you will still need to upload here. click “Save”

- Medical History Section must be filled out, click “Save”

- Parent/Guardian section must be filled out. Select your info from the drop down, click “Save”

- Signatures section must be filled out, click “Submit”

- After completion, click “view” in the Confirmation column and print out the “Confirmation Message”, parent needs to sign and also have your son sign and date it.

- Have your child drop off the Athletic Clearance Confirmation page to the Activities Office at school so they have a copy on record.

- Once everything is confirmed, the status will show “Cleared”
