For all request, including registering a new player, Casual player ($10 player), change of delegate, game time change, uniform change, new club request, request of minutes, umpire request etc.
One form to be completed by each team. Senior teams need each individual players to sign. School team waivers to be signed by the school principal.
To be completed for any senior player changing teams. Needs to be completed BEFORE the player plays for their new team.
