I led the transformation of a health department's PHAB accreditation process by designing a sustainable, department‑wide accreditation management system that eliminated crisis operations, standardized workflows, and strengthened cross‑functional accountability. Using a hybrid methodology grounded in project management, Lean Six Sigma, and change management, I built the tools, governance structures, and performance systems that now support 10+ divisions and ensure long‑term readiness for the 2028 reaccreditation cycle. This work generated measurable operational value, reduced rework, and positioned the department for national recognition.
This project showcases how I redesigned and scaled a high‑volume Food as Medicine program by integrating Agile project management, Lean process improvement, and culturally responsive nutrition education. Through workflow optimization, cross‑organizational coordination, and bilingual patient engagement strategies, I built a safer, more efficient, and more equitable distribution model that served 2,400+ families annually, eliminated traffic‑related safety incidents, and strengthened diabetes self‑management for high‑risk patients.
