All youth and registered adults must have BSA medical forms on file with the Troop in order to attend scouting activities.
These items must be updated and submitted to the Troop's Forms Coordinator annually.
Annual activity permission form (for youth only)
BSA Med Forms A, B1, & B2
BSA Med Form C (if attending an activity longer than 72 hours)
Copy of insurance Card

Needed for all Scouting activates. Example: a day trip or weekend campout).

Signed and dated by the Participate (The Youth).
Expires a year after it was signed and dated.
BSA Med Form
Only needed for overnight activities longer than 72 hours. Example: summer camp or high adventure.
Most be filled out by licensed physician.
Must be signed by licensed physician.
Must be dated by licensed physician.
Expires a year after it was signed and dated.
A copy of both sides of your Medical Insurance Card (Front & Back)
Can be a photo taken with your cell phone as long as it's legible and can be printed clearly.
General Notes
Avoid sending a generic picture of your forms with your phone. We will no longer be accepting these for the Troop's file. It will be better to deliver a paper copy if you don't have a way to create a clear copy or scan.
If you don't have the ability to scan and would like a digital copy, someone in the Troop may be able to scan it for you.
