Work permits are usually secured for the employee by the employer, but in certain cases, the employee may have to do some leg work. See below for information:

TO BE VALID: The Minor, the Minor’s Parent/Guardian, and the Employer must sign the permit. A Job must be in place before the permit is issued
A minor under the age of 14 is not permitted to work and may not be employed. Work permits are for minors 14 through 17 years of age. These minors may only work with a work permit.
The work permit must be in the employer’s possession before the minor is permitted to work.
If the minor changes jobs, a new work permit must be obtained.
