•  A completed Heath History Section 5 form (link below).

• A blank Physician Physical Section 6 form (link below).

Please wear:

•  Glasses/contact lenses (if required)

• Athletic closing 

• Sneakers

Physical participants are to enter through the Athletic doors across from the Tennis Courts.
