ROUTINE HOME CARE
Hospice Services are provided in the patient's home, wherever it may be.
CONTINUOUS CARE
Hospice Service is provided for a minimum of 8 consecutive hours for a period of crisis, and control of acute medical symptoms.
IN-PATIENT CARE
Patient care is delivered in a contracted skilled nursing facility, hospital, or sub-acute facility for symptoms management if symptoms cannot be controlled in the patient's home.
RESPITE CARE
Family members or caregivers can receive up to 5 consecutive days of respite care. Patient will be temporarily placed in a skilled nursing facility.
