Important Documents & Forms

Below are the required forms that must be completed by a licensed health care provider and uploaded into DragonFly in order to participate in Scholastic Athletics. 

Brunswick County has completed the transition to DragonFly to maintain athletic paperwork. Paperwork will no longer be accepted at the school. All sections of DragonFly will need to be completed prior to the tryout date in order for your student to be allowed to tryout. Only the Physical Examination Form and the Medical Eligibility Form must be uploaded AFTER they have been completed, signed and dated by a doctor or physicians assistant. The remainder of the requirements must be completed digitally online or within the App 

Students and parents are responsible for making sure all information is up to date and your registration is complete.
