Ask For a Complaint Document From Your provider Then hand it in to Your Provder and make Share They send you a copy by email so when they say that you did not put in a compliment with us you have it sent from there work email. 

if They Still dont went to Then Maybe think about going to the NDIS Commission or even giving them a Call but make sure you have a copy of the email that they sent to you with the Compliment forum on your phone or on your loptap
