Procedure for transfer cases:

a. Endorsement from current orthodontist (if possible)

b. Panoramic & Cephalometric xray (previous)

c. Ortho pictures (previous)

d. Photocopy of dentist notes (if possible)

e. Waiver for removal of brackets & continuation of ortho treatment

* Upon consultation, our orthodontist will advice the patient if he/she needs to have a new set of diagnostic records for treatment planing

* Our orthodontist & Staff has the right to defer cases with their discretion
