Three-dimensional image-guided adaptive brachytherapy (3D-IGABT) has transformed modern radiation oncology by enabling highly conformal dose delivery while minimizing irradiation of surrounding normal tissues. Advances in imaging, treatment planning, applicator technology, and quality assurance have created numerous research opportunities for medical physicists and radiotherapy staff.

Research Highlight

Advanced Brachytherapy Planning Strategies

Modern brachytherapy planning offers opportunities to improve target coverage and organ-at-risk (OAR) sparing through advanced optimization techniques. Research may focus on multi-criteria optimization, knowledge-based planning, robust optimization accounting for uncertainties, and artificial intelligence (AI)-assisted planning. The goal is to develop more efficient, consistent, and personalized treatment plans while maintaining high clinical quality.

Quality Assurance (QA)

Quality assurance is a fundamental research area in 3D-IAGBT because small uncertainties in source positioning, applicator reconstruction, or imaging can result in significant dosimetric errors. Research topics include applicator reconstruction accuracy in CT and MRI, source position verification using radiochromic film or detector systems, end-to-end testing of the entire treatment workflow, and MRI-specific QA such as geometric distortion assessment. These studies aim to improve treatment accuracy, safety, and consistency.

3D-Printed Applicator Design

Three-dimensional printing enables the development of patient-specific applicators and treatment accessories tailored to individual anatomy. Research topics include custom gynecological applicators, surface mould brachytherapy devices, interstitial needle templates, and material characterization. These studies aim to improve target coverage, reduce doses to healthy tissues, enhance procedural accuracy, and support personalized brachytherapy treatments.

In Vivo Dosimetry

In vivo dosimetry provides direct verification of delivered dose and serves as an additional safety measure. Research can evaluate technologies such as MOSFETs, OSLDs, TLDss, and plastic scintillation detectors for real-time dose monitoring. Studies typically compare measured doses with treatment planning system calculations, assess detector performance in high-gradient dose regions, and investigate methods for treatment delivery verification.

Emerging Technologies

Emerging research areas include AI-based auto-segmentation, automated treatment planning, adaptive brachytherapy, digital twin technology, and radiomics. These innovations aim to create more personalized, adaptive, and data-driven treatment workflows. Future developments are expected to integrate advanced imaging, machine learning, and real-time treatment verification to further improve treatment outcomes and patient safety.
