What is Hospice?
Hospice care is a type of treatment that focuses on aggressive symptom management for patients who are battling a terminal illness. SilverStone offers a team that supports the efforts of families and caregivers to help patients receive a higher quality of life. Hospice doesn’t mean giving up, and it doesn’t mean giving up hope. Our Board-Certified Medical Directors facilitate a caring environment and lead our team of nurses, aides, spiritual counselors, and social workers to provide comprehensive treatments tailored to meet patients’ needs and desires.

Routine In-Home Care
SilverStone Hospice provides in-home Hospice support so you can enjoy what matters most to you and your family.
What early assessment looks like:
- Our Administrator, Director of Nursing and a member of our Transitional Care Consultant team will connect with you and set-up the best time to visit with you and your loved ones.
- Our goal is to visit you in person within 3 hours to present your plan of care options.
- You choose which plan is best for you and your family.
- You will experience quick and timely support from our team for you and your loved one.
Hospice supports the entire family and helps improve the quality of life for the patient and their loved ones. Hospice care ensures that the patient is comfortable, free of pain and focuses on providing physical, emotional and spiritual comfort. We empower the patient to reduce frequent re-admissions to hospitals or ER visits. Hospices assist in the preparation and coordination of advance directives and decrease stress levels by offering comprehensive care of the mind, body and spirit.
Transitional Care Support
SilverStone Hospice, in partnership with our home health agency, is able to provide effective transitional care support for acute patients discharging from facilities or currently residing at home. Over the last year, we have been able to reduce unnecessary hospital readmission while providing personalized care in the home setting.
Services:
Care is initiated within 24 hours of discharge.
Additional Skilled Nursing visits.
Appointments scheduled with primary care physician or visiting physician within 14 days of discharge.
Remote monitoring for CHF and COPD patients.
Social Services for advance directive preparation.
Respite for Caregiver Relief
We empower families. By mirroring the family system, we naturally become an authentic extension of care, love and support. We enable families to confidently provide the best care experience for their loved ones.

Built a top a decade-old hospice organization, Silverstone was founded by Alfonso Montiel, a successful entrepreneur and vigil volunteer for hospice patients. Alfonso saw an opportunity for higher quality care within the hospice industry and set out to change the way people view hospice.
Inspired by the mission-led works of Dame Cicely Saunders, the renowned nurse, social worker, and physician who created modern-day hospice and palliative care, Alfonso set out to deliver hospice services through its original vision of incorporating the four pillars of care: physically, emotionally, spiritually, and socially. Hospice care that rewards the individual’s life.

SilverStone was created to honor our patients’ lives. With generous spirits and servant’s hearts, our team provides the gift of being present for our patients and their families.
Every day we help support the loving efforts of families and caregivers to help patients receive a higher quality of life and the care they deserve. To us, hospice doesn’t mean giving up, it means giving each a voice to the hope and desire of each person we serve.
A hospice volunteer is someone who has the passion and drive to selflessly care and advocate for people during their time of need. Someone who is able to see past the patient's diagnosis and be make meaningful connections to bring them joy. A hospice volunteer should have the passion to consistently visit patients on a regular basis.
Hospice volunteers are important to the core of the holistic care we provide. They provide companionship and emotional support to our patients as they transition. This could look like playing a board games together, reading a book together, and simply listening to the patient's stories. Our volunteers also take part in our "no one dies alone program" where they are specially trained to wait alongside our patients as their time of transition draws close.
The volunteers are at the core of helping us create an experience that simulates activities and environments our patients enjoy as well as acting as an advocate for our patients. Their goal is to bring reassurance that what matters most is not how much time we have, it is how we live each present moment.
Mental Health Wellness
Our communities will have the best care and support before, during and after death. We are fiercely compassionate as we vigilantly support our patients in their final hours so that no one dies alone.
We sit bedside and hold the hand of our patients as we read poems and listen to their favorite music. We master the art of being present and wait by their side as our patient’s transition. Our vigil team is always accessible to our loved ones and families, even in the most challenging circumstances.
