Age Requirement

Nursery

August - 4 years old by August 1 until October 31

Kindergarten

August - 5 years old by August 1 until October 31

Grade 1

August - 6 years old by August 1 until October 31

OR or must have COMPLETED kindergarten

Grade 7

have graduated from duly recognized public and private elementary school

Documents Needed

• Photocopy of Form 138 (current Report Card)

• Certificate of Good Moral Character (For Grade 7 and Transferees only)

• PSA Birth Certificate (Photocopy)

• Baptismal Certificate (Photocopy)

• Soft Copy of recent 2”x2” colored pictures

• Certificate of Completion duly signed by the Principal (for Grade 7 Applicants only)

• Certification if ESC grantee (For Grade 8 to 10 Transferees)
