Age Requirement
Nursery
August - 4 years old by August 1 until October 31
Kindergarten
August - 5 years old by August 1 until October 31
Grade 1
August - 6 years old by August 1 until October 31
OR or must have COMPLETED kindergarten
Grade 7
have graduated from duly recognized public and private elementary school
Documents Needed
Photocopy of Form 138 (current Report Card)
Certificate of Good Moral Character (For Grade 7 and Transferees only)
PSA Birth Certificate (Photocopy)
Baptismal Certificate (Photocopy)
Soft Copy of recent 2”x2” colored pictures
Certificate of Completion duly signed by the Principal (for Grade 7 Applicants only)
Certification if ESC grantee (For Grade 8 to 10 Transferees)
Procedures
1. Scan the complete requirements and send them to admission.sics1967@gmail.com.
Subject: [Grade Level] - [Surname, First Name]
Example: Grade 1 - Dela Cruz, Juan
2. Your email will be acknowledged. Wait for 2 to 3 days to assess the submitted documents.
3. The Guidance Office will schedule and conduct an interview (via Zoom) with the applicant for the
assessment. The presence of the parents/guardian are requested.
4. A Notice of Acceptance will be sent to your registered email once your child's application is approved
5. Updates on the schedule of enrollment will be posted on the Official Facebook Page or school website.
