You may download the Learner Enrollment and Survey Form (LESF) below and fill in the necessary information for enrollment. You may also use the online enrollment form below for convenience and attach the scanned copy of the requirements. Please bring the needed requirements upon enrollment. Requirements are listed below.

This enrollment survey shall be answered by the parent/guardian of the learner.
Please read the questions carefully and fill in all applicable spaces and write your answers legibly in CAPITAL letters. For items not applicable, write N/A.
For questions/ clarifications, please ask for the assistance of the teacher/ person-in-charge.

Duly accomplished Enrollment Form
The following steps are necessary during enrollment, please be guided accordingly.
Proceed to any available admission officer.
Take the Pre-test upon admission.
Undergo an interview with an admission officer for the final selection of your track.
Have your documents/requirements presented to the evaluation officer for checking.
Fill-up the enrollment form completely, correctly and legibly.
Submit to enrolling officer the duly accomplished enrollment form together with the requirements attached.
