Every player must fill out the Pre-Season Roster Form.
Print your other completed forms and turn in to the Shanley/Sacred Heart Activities’ Office.
Please keep a copy for yourself and also give copies to your home school’s office.
Shanley/Sacred Heart students will go to ParentWeb for all items except the physical.
How to fill out the Pre-Season Roster Form:
Complete the boxed chart with the player’s information.
The Paperwork and Physical boxes on the form require YES/NO responses.
The Paperwork box is only for Co-op students and refers to the 2 attached Co-op forms: Co-ops Receipt and Acknowledgments Sheet and Co-curricular Activities Form.
If you are a Shanley student, fill out the Paperwork box with "N/A".
