What type of need?

The majority of physical health-related needs are managed by the school nurse. 

This includes:

• Allergy Plan

• Asthma Plan

• Diabetes Plan

• G-tube/feeding Plan

• Ostomy Plan

• Oxygen/respiratory support

• Seizure Plan

• Tracheostomy Plan

• Urinary Catheters

• Vision

Call the school and ask for the direct contact for the school nurse.

Suggested script:

"Hi, my name is [insert name].  I am a [provider role] at UCSF and am requesting to speak with the school nurse to obtain more information about a student we serve at UCSF.  May I please be transferred to speak to them, or can you provide direct contact information for them?”

Specific pathways exist for other mental health, neuro/developmental/psych, and social/logistic needs.
