1. First Level:

   • Elementary/Middle School: Social Worker

   • High School: Social Worker and/or Academic Counselor

2. Second Level:  

   • School Site Administrator (Principal or Vice Principal)

If child does not have an IEP:

1. First Level: Social Worker

   • Middle/High School: Special Education Department Chair

2. Second Level:  

   • Elementary School: School Site Administrator (Principal or Vice Principal)

   • Middle/High School: School Site Administrator (Principal or Vice Principal)

If it is unknown whether the child has an IEP or not:
