Primary Care

Primary Care Division Our fourteen (14) health centers/programs, provide high-quality, compassionate care to the City’s children, youth, adults, and elders regardless of immigration status or ability to pay. Services include prenatal care and family planning services; integrated behavioral health and substance use services; dental, gender health, nutrition, pharmacy, and podiatry services; integrative health services such as acupuncture; and care for people experiencing homelessness and refugees/asylum seekers. We are a mission driven organization dedicated to eliminating heath disparities and becoming an anti-racist organization.

Forensics

Jail Health Services (JHS) maintains four County Jails and provides a comprehensive and integrated system of medical psychiatric and substance abuse care to prisoners at the San Francisco County Jail system Additionally the San Francisco Sheriff's Department maintains a locked security unit at Zuckerberg San Francisco General Hospital and Trauma Center for provision of inpatient services to patients whose medical/mental health problems require a higher level of care than is available in an outpatient setting.

Laguna Honda Hospital and Rehabilitation Center (LHH), located on 62 acres of parkland west of Twin Peaks, is one of the largest and most modern skilled nursing facilities in the country. It is San Francisco's flagship rehabilitation, post-acute care, short stay, and long term facility.

Maternal, Child & Adolescent Health⚕️

The Maternal, Child and Adolescent Health (MCAH) Section of the San Francisco Department of Public Health focuses on the most vulnerable children and families, filling what would otherwise be a serious public health gap. We assess the needs of MCAH populations, and identify and address urgent MCAH issues in collaboration with key partners. The work of MCAH is critical to protecting and promoting the health of San Francisco women and children. We aim to reduce health disparities and improve health outcomes by strengthening the public health systems and services that address the root causes of poor health.

Community Health Programs for Youth⚕️

Community Health Programs for Youth (CHPY) provides a full array of primary care, counseling, and confidential services for youth and young adults between the ages of 12 – 24 through school based clinics and in partnership with community based organizations across San Francisco.

We are dedicated to eliminating health disparities and becoming an anti-racist organization by prioritizing racial justice and equity in healthcare. We are striving to promote an inclusive work environment that is aware of and responsive to bias and privilege. We aim to build trust among staff from different backgrounds, provide opportunities for professional development and advancement, and actively recruit and retain a diverse work force at all levels.

Under general nursing (Lead Clinician) and medical direction (Medical Director), utilizing standardized procedures, the Physician Specialist functions within an organized system for the delivery of health care in association with other members of the multidisciplinary health care team and performs specialized provider duties including offering clinical consultation, in clinical settings of the Community Health Programs for Youth.
