Castro Mission Health Center

Castro Mission Health Center (CMHC) serves approximately 4000 adults and children. Approximately 50% of our patients are Latinx and many of our patients are LGBTQ identified. We have longstanding and active HIV treatment and prevention programs. We are a leading provider of Transgender care and also host a weekly Transgender youth clinic called Dimensions. Over the last few years, we have expanded our pediatric and prenatal services and now care for a growing number of families.

We have an energetic and supportive provider group of 5 Family Medicine Physicians and 2 Family Nurse Practitioners. We provide high quality team-based care and integrated behavioral health services. Our clinical and office staff are experienced and engaged. The majority of staff are bilingual and many are bicultural. CMHC staff enjoy socializing together and try to make wellness a priority in our daily work.

Chinatown Public Health Center

Chinatown Public Health Center (CPHC) has been serving San Franciscans and the Chinese community since 1929. Since breaking ground in 1969 on the current and fifth site in Chinatown, CPHC transformed itself from a traditional public health center into a full-service, community-oriented, patient-centered medical home. CPHC offers a full array of services including primary care for adults and children, prenatal care, integrated behavioral health, pediatric dentistry, community health education, and Women, Infant, and Children Nutrition Supplemental Services. CPHC serves close to 6,000 active patients, many of whom are socioeconomically disadvantaged and/or culturally isolated, and serves clients with cultural sensitivity and pride.

️Community Health Programs for Youth⚕️

Community Health Programs for Youth (CHPY) provides a full array of primary care, counseling, and confidential services for youth and young adults between the ages of 12 – 24 through school based clinics and in partnership with community based organizations across San Francisco.

️Curry Senior Center⚕️

Curry Senior Center (CSC) is a joint project of the San Francisco Department of Public Health (SFDPH) and Curry Senior Services, a private, nonprofit organization. From its key location, it serves the persistent and urgent needs of low-income seniors who are aging in place in the Tenderloin and South of Market districts of San Francisco. It provides services to seniors that promote independent living while maintaining their dignity and self‐esteem. The Curry Senior Center serves about 1,600 patients per year in the area of Primary care and preventative health care services and the target population is seniors ages 55 years and older.

Maxine Hall Health Center⚕️

Maxine Hall Health Center (MHHC) serves approximately 3400 patients from across San Francisco. Located in the City’s historic Fillmore neighborhood, MHHC has long been dedicated to serving the Black and African American residents of the community as well as immigrants from Asia, Mexico, Central America, Latin America, Africa and the Middle East. Our diverse patient population motivates our very diverse staff to do our best work every day. We work as a multidisciplinary team and value continuous learning as we strive to constantly improve the care we give to our patients.

Ocean Park Health Center (OPHC) has been serving the Sunset District and surrounding neighborhoods for over 50 years. We strive to provide high-quality, evidence-based primary care and preventive health services to our patients.

Potrero Hill Health Center⚕️

Potrero Hill Health Center (PHHC) provides primary care, prenatal, pediatric, behavioral health, substance abuse, and family planning services to approximately 3,500 patients. We also offer acupuncture for all patients in the health network. We serve a racially and ethnically diverse population with about one-third Spanish speakers and one-fourth identifying as Black/African American. The PHHC has a small clinic feel, and a cohesive and supportive staff culture.

Silver Avenue Health Center⚕️

Silver Avenue Family Health Center (SAFHC) serves a diverse patient population, including those who identify as Latinx, Asian, and Black/African American, and delivers full spectrum family medicine care to patients, including children and pregnant women. Providers take pride in offering high quality, compassionate, culturally competent, evidence-based care to the families we serve.

Southeast Health Center

Southeast Health Center (SEHC) is a full-service health clinic that provides affordable, comprehensive and quality care to people of all ages. Located in the Bayview Hunter’s Point neighborhood since 1979, we provide full spectrum Family Medicine care for common illnesses, diabetes, high blood pressure, and sexually transmitted diseases (STDs). We provide additional services such as confidential HIV testing and counseling, pregnancy testing, prenatal care, mental, and vision care.

Tom Waddell Urban Health Center

Tom Waddell Urban Health Center (TWUHC) and our community based locations is SFHN Primary Care’s flagship Health Care for the Homeless program. We serve adults experiencing homelessness, residents of permanent supportive housing, and other highly vulnerable individuals at programs in and around San Francisco’s Tenderloin district. Our patients face challenges from chronic disease, substance use, mental illness, poverty, social isolation, systemic racism and bias, and a history of multiple traumatic life events. Our patients are highly-diverse and majority Black, Indigenous and People of Color. Services are aligned with trauma-informed and harm reduction models. Specialty services include comprehensive HIV prevention and care; community-based Hepatitis C treatment; office based opioid treatment; transgender and gender-expansive care; integrated behavioral health services; and integrative pain management.
