The impact of supply disruptions in a social marketing program for reproductive health on contraceptive use and pregnancies in Zambia. [JMP]

Abstract

Social marketing programs, often supported by external donors, use commercial marketing techniques, such as product branding and subsidized pricing, to enhance demand for contraceptives. In Zambia, a longstanding social marketing program experienced a funding disruption between September 2014 and August 2016. Using contraceptive calendar data from the 2018 Zambia Demographic and Health Surveys, this study assessed the impact of the disruption on modern contraceptive use, method mix, and pregnancy outcomes. Employing difference-in-differences and synthetic control approaches, I compared trends in Zambia before and after the disruption with those in 12 sub-Saharan African countries that did not experience similar disruptions. The program halt did not significantly affect overall pregnancy rates. However, the result showed declines in short-acting contraceptive use, with no evidence of substitution towards medium-term or long-acting methods. Oral contraceptive use declined by 0.4–0.5 percentage points and condom use by 0.2–0.6 percentage points. Reductions varied across subgroups. For example, the decrease in condom use was greater among adolescents, urban residents, and unmarried women. These findings highlight that even mature programs can face rapid reversals in contraceptive use when reliable alternatives and domestic financing are lacking. National governments and donors should invest in sustainable supply chain systems for essential reproductive health commodities and plan cautious, well-sequenced transitions when phasing out externally funded projects.
