(make sure to click the "GOT IT!" boxes to acknowledge you have read and understand the tournament rules).
PLEASE NOTE: Your entry is complete when BOTH payment and questionnaire responses have been received.
REFUND/CANCELLATION POLICY: Requests to withdraw/cancel must be made in writing to tournament management. Upon receipt of written intent to cancel, a refund of the entry fee shall be made to the bowler's original payment method less a $50 service charge. (100 percent of this fee is added to the tournament prize fund). There are NO refunds for cancellations or withdrawals received on or after January 17, 2026.
