ATHLETIC CLEARANCE
All SDA students are encouraged to try out for a sports team. In order to try out, the athlete must complete the following clearance process. Click here for instructions on using homecampus.com
Go to www.homecampus.com and complete the online clearance for the 2024-25 school year (do not use last year's clearance).
Submit a picture of health insurance card either via email to megan.thompson@sduhsd.net or drop off at the Athletic Office.
Submit a physical performed by an MD, DO, PA, or NP. The physical must not be older than one calendar year and must include the provider's signature on this form: Download Physical Form Here. Only the Medical Eligibility Form (page 4) should be returned to the school site.
Use of Private Automobile Form. All athletes and parents who drive when District transportation cannot be utilized must submit a form in their name.

Insurance
The San Dieguito Union High School District does not provide medical insurance coverage should an athlete get hurt during a school activity. However, the District does make student-accident insurance available if the athlete does not have insurance. Please visit www.peinsurance.com
PE Credit for Athletics Option
PE Credit for Athletics applications will not be accepted until the start of athletic activities. Application for the 2024-25 school year will be posted here in August.
A student can earn 5 PE credits per season with a maximum of 20 credits earned through participation on a SDA sports team. The option is available to grades 10-12. The FITNESSGRAM must be taken the year prior to applying for credit. Credits will not be given retroactively. For FITNESSGRAM Performance Standards, please click here.
The due date for PE Credit for Athletics applications will be:
Fall - 9/15
Winter - 12/15
Spring - 3/15
