SCREENINGS AND CLOSED LOOP REFERRAL WORKGROUP

The Coos Coalition for Young Children and Families Screenings and Closed Loop Referrals Workgroup is responsible for ensuring universal child developmental and caregiver depression screenings and for improving family referrals to services through the development of a closed loop referral system. The Coalition Leadership Team has approved the Unite Us platform for this purpose.

The purpose of the Screenings and Closed Loop Referrals workgroup is to:

• Continue to promote annual developmental screening for children 0-5

• Continue to promote timely screening for maternal and caregiver depression

• Improve systems coordination by piloting a closed loop referral system using the Unite Us platform

• Provide a report on lessons learned through the pilot to the Coalition Leadership Team
