FWISD 2022 STUDENT DEVICE REPORTING
Students grade 4-12 use this form for device request. Students grade PreK-3rd grade may need an adult to help them. When a student requests a new device for any reason, the student completes this form, and their original device is collected in TipWeb. Record the asset number of the returned device below.

Lost | Stolen | Found | Transfer Form for FWISD Devices
Serial numbers must be entered in order for your request to be completed.  This form must be completed by a campus device administrator, facilitator, or campus technology liaison. This is a one stop shop for reporting Lost, Stolen, Found, and Transfer Requests for Student Devices. This form may be used to request teacher device transfers by the campus receiving the device.  If necessary, after completing the form, please update TipWeb with the proper status and room location. This form is to be completed only by the device administrators and facilitators. Do no share this form with students.

In addition to the form below, students grade 4 - 12th need to complete the Student Device Reporting form after logging into their fwisd account. The link to the form is: https://bit.ly/sdr22.  Students grades PreK - 3rd can be assisted by adult staff members. All student device repairs must be submitted in TipWeb as well.
