Self Employment form to complete if you are Self Employed (Uber, Lyft, Food Delivery, your own business, etc)
If you get tip income and get a W-2 from an employer, download and fill out this form.
NEW FOR 2025: If you get tip income and are self employed, download and fill out this form.
NEW FOR 2025: If you were paid for overtime work, download and fill out this form.
NEW FOR 2025: If you bought a new car with a car loan in 2025, download and fill out this form. Bring the VIN (Vehicle Identification Number) of the vehicle.
Other items to bring with you
Your ID(s) and Social Security Card(s).
IRS letters (or your records) of payments you have received (Child Credit advances, Economic Impact Payments, PIN, etc.)
A copy of last year's tax return.
All of your tax documents including any documentation on self employment or business income and expenses.
NEW FOR 2025: If you receive overtime pay for working more than 40 hours per week, bring in your calculations.
If you receive health coverage through the ACA, bring form 1095-A ( download from your covered California online account).
A blank check for your account (required for direct deposits).
If you plan to Itemize, bring all of your information for Itemizing.
Also, if you are applying for a property tax rebate or a rent rebate, then bring your receipted property tax bill(s), or your landlord's certification of rent receipt.
PLEASE open all envelopes containing tax documents, and organize your documents in the following order: SSA/RRB, W-2s, 1099-Rs, 1099-INTs, 1099-DIVs, Broker statements, .....
