New Patient Process
To become a new patient at our clinic, all prospective patients are required to book a Meet and Greet appointment by calling our clinic. This initial visit gives us an opportunity to review your medical history and determine whether our clinic is the right fit for your ongoing care. Please note that acceptance as a new patient is not guaranteed and is subject to the physician’s discretion.
Behavior Policy
Our clinic maintains a Zero Tolerance Policy for any form of inappropriate or disrespectful behavior. All patients, visitors, and staff are expected to conduct themselves in a respectful and courteous manner at all times.
Any form of verbal abuse, harassment, aggression, or disruptive behavior will result in immediate removal from the premises and may lead to permanent discharge from the clinic.
Forms
Please inform our receptionist of any forms you wish to be filled out prior to your appointment as most forms are not covered under insurance and is subject to fees. Our receptionist will provide more information on the fees or please visit the Forms & Fees tab for more information. Patients are required to pay up-front and we accept cash.
