New Patient Instructions

To become a new patient, please call our office to confirm if we are in network with your insurance plan, and we currently serve patients 18 and older. We are accepting new patients and require the last two office notes of a previous provider. If you have seen a doctor within the last five years, you could go to their office or  come into our office to complete the Medical Record Release form. As soon as we receive the records, we can schedule your new patient visit. 

Please arrive 15 minutes early to your new patient visit with your insurance card and photo identification. Please review your insurance card to see if you have a doctor listed on your card. Please call your insurance company to confirm that your new PCP would be Dr Serrano prior to your new patient visit to avoid delays.
