☑️ For Intellicare/Avefa Patients (Adult) Please Call or Text Us for Appointment.
Safe and Orderly Out Patient Visit
☑️ Upon arriving, Fill-up the Health Declaration form & Patient Information Sheet at the Pre-Triage Area.
☑️ Once accomplished, Proceed to Triage and Submit the forms to the Nursing Attendant on Duty.
☑️ They will then assist you to move to the Out-Patient Information for Queuing (or to HMO Section for Approval).
☑️ **For Patient with Cough, Colds or Any Covid-19 Symptoms, you will be checked by our Physician at the Tent Area.
