The MHSAA physical form is a two-page document required for all student-athletes participating in high school sports in Michigan; it includes a medical history section for parents/guardians or 18-year-olds and a physical examination section to be completed by a medical professional. For the 2026-27 school year, the physical examination must be completed on or after April 15, 2026, to be considered valid.

Once completed and signed by a physician, this form can be emailed to muthphysicals@gmail.com.  You can scan the form or take a picture of both pages to submit it via email.

Although email is the preferred way to submit the form, you may also bring a physical copy to turn in the first day of practice.  DO NOT upload the form to FinalForms or TeamSnap, it must be emailed directly to muthphysicals@gmail.com.

This form is due by the first day of practice.  IF WE DON'T RECIEVE A COMPLETED AND SIGNED PHYSICAL FORM YOUR CHILD WILL BE UNABLE TO PARTICIPATE!
