We are excited to announce that all SBISD athletic paperwork is now submitted online! In order to complete this process, you must know the student's school ID and enter the student's name exactly as it appears in Skyward (including double last names, hyphens, etc.) The only form that will be accepted in paper format is the physical (see link below.)
You must have an updated physical on file every year. Physicals must be dated after
APRIL 1 to be valid for the following school year.
If you have asthma, an allergy requiring an epi-pen, or diabetes, you must also have your doctor complete the appropriate supplemental forms.
