Instructions

Fill out the form completely.

Click on the 'Submit' button only once.

Wait till you get the confirmation message below the Submit button. It may take a few seconds.

Do not close the browser or click on Back button until you get the confirmation message.

Once successfully submitted, you will get an email message on the email address you provided. It will have a copy of your form attached as PDF for your record.

Check you Spam folder if you didn't get the email message within a few minutes. 

Once you see the email message, no further action is required. We will process the form and let you know when the account is updated.
