OFA THYROID TESTING

COST for all Borzoi: $180 (OFA fee included)

TIME: 12:00 pm to 4:30 pm

LOCATION: OAK Room (tentatively)

Skip the line! To skip the line of drop-ins, please fill out your forms and pre-pay.

Here is what you need to do to skip the line:

1. Download BOTH Forms for each dog you are testing:

OFA THYROID FORM

MICHIGAN STATE UNIVERSITY FORM

2. FILL out both forms for each dog you are testing

3. EMAIL both forms (PLEASE INDICATE “Borzoi Thyroid Clinic” in the subject line) to horsesensevet@gmail.com

Once you submit your completed form to Dr. Brendel’s office, you will receive an invoice from Horse Sense Vet LLC and will be able to pre-pay for your test by using a credit card.

Drop-ins will be available on the first-come, first-serve basis. 

Please fill in your OFA paperwork ahead of time to expedite the process. See the links above for the forms.  Also, printed forms will be available at the clinic for drop-ins. Please make sure to have your dog’s registration number as well as the registration numbers of sire and dam.
