Database Submission Instructions
General Information
The database submission form has required fields that must be completed to submit the form. Those fields will be identified by a red (*) next to the title.
When answering a question, provide as much detail as possible. There are several fields provided for comments, notes, and additional information if needed.
Section 2: Patriot Information
Last Name/AKA - Use the SURNAME the Patriot to be known by. Keep in mind there may be numerous ways to spell a Surname, Surnames may have been changed, or spelled phonetically. Add any additional known names possibly used by the Patriot in the Last Name AKA field.
First and Middle names should be listed as recorded on any supporting documents.
The Suffix field supports any title or position used by or given to the Patriot. LIst any known or possible Suffixes that may have been used.
In both the Birth and death fields: provide as much information as possible to assist in the search for the Patriot first known residence and history.
Death fields: Again provide as much information as possible as the Patriot may have moved to a new location for land and a better way of life.
Rank and service information should be as detailed as possible. Also list the source of the information if possible in the same paragraph.
Section 3: Cemetery Information
Cemetery name/AKA - List the name if known. If the burial site does not have a specific name, use the “Cemetery Notes” section to describe the burial location.
If the location can not be identified by street address or GPS coordinates, driving directions should be as detailed as possible. Some burial locations are simply an abandoned location in the woods which may be difficult to find.
Private Property is a key issue for locating and visiting burial sites. Many family burial sites may be on what is now private property, Please make every attempt to determine and identify the status of the burial site. You can use the “Cemetery Notes” section to provide additional information on property status to reduce liability for those who may wish to visit the burial site. If unsure, error on the side of caution.
Find-A-Grave, if the Patriot’s grave site location is unknown but known to be in a specific cemetery, list the cemetery information if available and provide a link to the Find-A-Grave website if available.
Section 4: Gravesite Information
General documentation of gravesite.
If photographs are taken, list the number of photographs taken in the “Notes” section.
Section 5: Organizational Marker
If the “Marker Placed” question is “No or Unknown” leave the other fields blank.
The last field is for “Additional or Supporting Information”. If you have additional marker information not covered by the sections questions, please add your comments.
Section 6: Submitter Information
All personal information collected during the submission of your Patriot data is not sold or distributed publicly. The only required information is your name and email address to maintain communication related to your submission.
