Bring your Picture ID (State issued ID or Drivers License or Passport) and your health insurance card; if you are covered by two plans, bring both cards and determine in advance which is your primary coverage. Check your insurance plan booklet to see if you need a physician referral and/or authorization number before your visit.

Before you decide on your physical therapy outfit, it’s important to consider what part of the body you are having treated. Your physical therapist will need to have a good view of the body part in question, so if you’re having physical therapy on your knee, leg, or ankle, it’s important to wear shorts. Here are a few common clothing items that would be good options for a physical therapy appointment:
Workout clothes (gym shorts, yoga pants, t-shirts, tank tops, etc.). Many patients come to their appointments in their workout clothes.
Tennis or gym shoes.
Hair binder. Patients with long hair may need to pull their hair back into a bun or pony-tail – especially those patients undergoing shoulder or neck therapy.
Ultimately, you should wear something that makes you comfortable. However, there are a few things you should avoid outright.
What to Avoid
While comfort is important, there are some limitations to appropriate attire for your physical therapy session. Here are some items you should avoid:
Your work clothes. If you come to your appointment directly from work, bring some clothes to change into when you get to your appointment.
Clothes that are too tight. You’re going to be doing a lot of moving and stretching – make sure you’re wearing loose fitting attire.
Dress shoes or boots.
Excessive jewelry (necklaces, bracelets, etc.).
