ALL PROJECTS MUST BEGIN WITH THE COMPLETION OF THE ISEF PAPERWORK

If you are doing a survey or using human participants
- You need to fill out Form 4,
- You need to fill out the Sample Informed Consent
- The survey must be completed to approve the project
- Survey best practices
More details on the forms can be found on the GSEF or ISEF Website
The forms should be filled out and signed before any research takes place. (Only Forms 2C, 5B, 7, and the abstract are done after the research.) The dates of the signatures reflect when the approval or consent is given. Use MM/DD/YY format for all dates.
NOTE: BSL1 and BSL2 Checklist are not forms to be completed, they are just guidelines for those that are working with bacteria.

The International Rules & Guidelines include the forms necessary to document adherence to the rules.
These forms constitute written documentation of what will occur, or in some cases, has already occurred, in a research project.
They are designed to provide the information that is needed to review the project to ensure compliance with the ISEF rules and with laws and regulations that apply to the project.
The purpose of these rules is to:
protect the rights and welfare of the student researcher
protect the rights and welfare of the human participant
ensure adherence to federal regulations
ensure use of safe laboratory practices
protect the environment
determine eligibility for competition in the local, state fair
