Where are you located?
I offer in-home pelvic floor physical therapy services within a 30 mile radius of Luther, OK. I offer in-clinic pelvic floor physical therapy at Hillside Wellness House located at 111 W 1st St, Chandler, OK 74834.
Do I need a referral?
No. A referral is not needed. Dr. Rachel will work with you or your provider to receive appropriate documentation as needed within 30 days of treatment per Oklahoma practice guidelines.
