The Pithari Longevity Index app is developed by Ikaria Longevity + Concierge ("we," "our," or "us"), a physician-led longevity clinic founded by Dr. Brett Haake and Dr. Felice Haake in Vero Beach, Florida. Inspired by the Greek island of Ikaria, one of the world's five Blue Zones where people live extraordinarily long and vibrant lives, our practice is rooted in the science of aging well through prevention, optimization, and personalized care.

This Privacy Policy explains how we collect, use, and protect your information when you use the Pithari Longevity Index mobile application (the "App"). We designed this App with privacy as a core principle. Your health data is stored locally on your device and is never transmitted to external servers or shared with third parties.

1. Information We Collect

The App collects and stores the following information locally on your device:

• Account Information: Username and password (your password is stored in encrypted/hashed form and cannot be read in plain text by anyone)

• Profile Information: Profile photo (optional), membership date

• Health and Biomarker Data: The health metrics you voluntarily enter into the App, including but not limited to: VO2 max, grip strength, sit-to-stand repetitions, InBody score, ApoB, Lp(a), HOMA-IR, hs-CRP, triglyceride/HDL ratio, eGFR, ALT, uric acid, blood pressure, and smoking status

• Calculated Scores: Your Pithari Longevity Index score, domain scores, and score history over time

• Demographic Information: Age range and sex, used solely for age- and sex-adjusted percentile calculations

2. How Your Data Is Stored

All data is stored locally on the device where the App is installed. We do not operate external servers, cloud databases, or remote infrastructure that receive or process your personal or health information over the internet. Your data is not transmitted off the device.

3. Healthcare Provider Access

The App includes an administrator feature designed for use by the physicians and clinical team at Ikaria Longevity + Concierge. As part of your care, authorized providers using the App on the same device can:

• View your Pithari Longevity Index scores and biomarker data

• Review your score history and track your progress over time

• Enter biomarker test results on your behalf following lab work or clinical assessments

This feature supports the personalized, direct-access care model that Ikaria Longevity + Concierge is built on. Administrator access is protected by a separate credential and is only available on the device where the App is installed. Your data is not accessible remotely or over the internet.

4. How We Use Your Information

The information you enter is used within the App to:

• Calculate your Pithari Longevity Index score across five health domains

• Display your score breakdown and identify areas for optimization

• Track your progress over time so you and your physician can measure improvement

• Provide evidence-based recommendations to help you improve your healthspan

• Authenticate your access to the App

• Support your clinical care by giving your healthcare provider a clear view of your longevity profile

We do not use your data for advertising, marketing, analytics, or any purpose other than supporting your health optimization and clinical care.

5. Data Sharing

We do not share your data with third parties. Specifically:

• We do not sell your personal or health information

• We do not share data with advertisers

• We do not share data with analytics providers

• We do not share data with data brokers

• We do not transmit any data to external servers or services

The only individuals who may view your data within the App are the authorized physicians and clinical staff at Ikaria Longevity + Concierge, as described in Section 3 above.

6. Data Security

We take the security of your information seriously:

• Passwords are stored using cryptographic hashing with unique salts and cannot be read in plain text

• The App requires a gate password for initial access, providing an additional layer of security

• Administrator access requires a separate credential known only to authorized clinical staff

• All data is stored in the App's sandboxed storage, protected by iOS security mechanisms

• Profile photos are stored locally in the App's secure documents directory

While we implement reasonable security measures, no method of electronic storage is 100% secure. We encourage you to use a strong, unique password for your App account and to protect access to your device.

7. Children's Privacy

The Pithari Longevity Index App is not intended for use by children under the age of 13. We do not knowingly collect information from children under 13. If you are a parent or guardian and believe your child has provided information through the App, please contact us and we will assist you in deleting that information from the device.

8. Your Rights and Choices

Because all data is stored locally on the device, you have control over your information:

• Access: You can view all your data within the App at any time

• Deletion: You can delete all your data by deleting the App from the device. This action is permanent and cannot be undone.

• Correction: You can enter updated test results at any time. Previous entries are retained in your history so you and your physician can track your progress.

9. Third-Party Services

The App does not integrate with any third-party services, SDKs, analytics tools, advertising networks, or social media platforms. The App does not contain any tracking technologies.

10. Changes to This Privacy Policy

We may update this Privacy Policy from time to time. If we make material changes, we will update the "Effective Date" at the top of this policy. We encourage you to review this Privacy Policy periodically. Your continued use of the App after any changes constitutes your acceptance of the updated policy.

11. Clinical Foundation and Disclaimer

The Pithari Longevity Index was developed by the physicians at Ikaria Longevity + Concierge, drawing on board-certified expertise in family medicine, obesity medicine, anti-aging medicine, and functional medicine. The index integrates 14 biomarkers across five clinically relevant domains to provide a comprehensive picture of your physiologic reserve, metabolic health, and longevity trajectory. The scores and recommendations are grounded in peer-reviewed research and designed to help you and your physician identify optimization targets and track meaningful progress over time.

The App is a wellness and educational tool designed to complement your clinical care, not replace it. The Pithari Longevity Index is not a regulated medical device. Individual health decisions should always be made in partnership with your healthcare provider, who understands your complete medical history and can tailor recommendations to your unique needs.

12. California Residents

If you are a California resident, the California Consumer Privacy Act (CCPA) provides you with additional rights regarding your personal information. Because we do not sell or share your personal information with third parties, and all data remains on the device, the typical CCPA provisions regarding data access requests and opt-out rights are addressed by the local-only nature of our data storage.
